INSTRUCTIONS FOR MANDATORY DOCUMENTS, EQUIPMENT, AND TIMESHEET SUBMISSIONS

Prior to employment with DOGWATCH SECURITY AND TRAFFIC CONTROL LLC, the following COMPLETED documents must be submitted:

• Copy of Credentials
• Completed W9 Form
• Copy of NHI Certificate
• Signed Zero Tolerance Form
• Signed Independent Contractor Information Form
• Signed Payment Processing Form
• Signed Hold Harmless Agreement Form

How to Log on and take National Highway Institute Certification Course:

• At Google search bar type in National Highway Institute
• Create a Login
• Go to Course Search and type in: FHWA-NHI-133119
• Add that Course to Cart
• Complete the Course
• At the very end of the course you will receive a Certificate
• EMAIL ME THE CERTIFICATE
